How have the last two weeks been?

Nine short questions about the past fortnight, one at a time, and a last one about how much all of it has got in the way. For each one you choose how often it has been true. There are no right answers.

This is a self-report screening questionnaire, not a diagnosis. It cannot replace a face-to-face assessment by a qualified professional. If you are finding things significantly difficult, please speak to one.

The questionnaire used here is the PHQ-9 (Spitzer, Williams, Kroenke and colleagues), a nine-item self-report measure of depressive symptoms over the past two weeks. Item wording follows the published instrument exactly.
