All three self-checks

Self-check · about 5 minutes

How have the last seven days been?

Twenty-one short statements about the past week, one at a time. For each one you choose how much it applied to you. There are no right answers, and nothing here is a test you can fail.

  • Your answers stay in this browser tab. They are never sent anywhere.
  • Nothing is saved once you close the tab, and no account is involved.
  • You can go back and change any answer before the end.

This is a self-report screening questionnaire, not a diagnosis. It cannot replace a face-to-face assessment by a qualified professional. If you are finding things significantly difficult, please speak to one.

The questionnaire used here is the DASS-21 (Lovibond & Lovibond, 1995), a widely used self-report scale for depression, anxiety and stress. Item wording and scoring follow the published instrument exactly.
